To download a PDF Form from the DFSC Website:

Make sure you have Adobe Acrobat viewer

Click the link.  This will automatically launch the Acrobat Reader.

To download a .exe form from the Dallas FSC website:

Click the hyperlink to the file.
Save the file to your hard drive. Remember the location you saved it to.
Open the file.
You will get a window that looks like this:

You may click the "BROWSE" button to save the file to a selected location on your hard drive (Recommended).  Otherwise, the file will be saved to your Default Temp file. 

Once you select the location to save the DFSC file to, click the "UNZIP" button.  You should get a dialog that says "1 File successfully unzipped".  Click "OK", then click the "CLOSE" button.

The DFSC form will then be saved on your hard drive in the location that you selected.
